I am in the process of reformating/rewriting my resume according to the MT guidance.

I have a couple of questions:

1)   I worked in a restaurant for about 6 month between jobs.  It goes on the resume per MT rules -- no problem.  I assume that I don't need to include any major accomplishments?

2)  I left Company A for about a year to help start up a call center at another company, only to go back to the Company A working in the same job.  Can I lump the two together in a single block or do I need to set it up

Comp A job

Comp B job

Comp A job -- same description

3)  I'm going back to add month and year on all the old jobs.  I will try to get as close as I can, but it may be plus/minus two months from the actual date.  No intent to deceive, but it is difficult to recall event that happened 10 years ago.

1.  Unless there is an accomplishment that relates to the job your are applying for, just list it.

2.  So they understand the flow of your work experience, have it in 3 sections as you list it here.

3.   That's understandable.  If you have your tax returns or paycheck stubs from those employers, you could look at those to help job your memory.
